Adult Rehab Services Annex

The Adult Rehab Program is a psychosocial rehabilitation program which is designed to promote recovery and provide support for adults who experience severe mental illness. 

Adult Rehab offers case management, daily living skills training, outreach, advocacy, crisis respite, transitional living, supported housing, medication monitoring, supportive therapy, group therapy, interpersonal skills training, housing assistance, prevocational skills dvelopment, transporation, health education and a substance awareness group, as designated by individual treament plans. 

The Adult Rehab Program is located in The Annex on HillFair Court.  Entry into this program is based on a screening evalution.
